Registration and Medical Records
If you want to register at the practice, you will be required to fill in a form to request your medical records from your previous doctor. Please come in to pick up a form as soon as possible, rather than waiting until you are unwell to register or download a copy here.
Once you have registered at the practice you can see any doctor here.
We only see you without records in emergencies. We share information with other health professionals on a ‘need to know’ basis, only and would need your prior consent to disclose information to others, e.g. insurer, employer, lawyer.
In The Unlikely Event
In the unlikely event, that patients persistently and repeatedly ignore their own responsibilities to us and to other patients, we have the right to remove them from our register.
Similarly, patients who are violent or seriously abusive towards any of the practice staff and other patients, will be removed from our register.
